Crisis, Make Up in the anime
Crisis, Make Up (クライシス・メイク・アップ Kuraishisu meiku appu) is the command used with the Holy Grail in order for Sailor Moon to transform herself into Super Sailor Moon. It debuted in episode 111 of the anime and Act 30 of the manga.
Anime
When Usagi is already in her Sailor Moon form, she holds the Holy Grail. It's lid opens, revealing bright light, transforming Usagi into Super Sailor Moon.

Trivia
- When this transformation was first used in episode 111, Sailor Moon did not shout the words, "Crisis, Make Up".
- In episode 125, a different sequence was used for this transformation.
