"Chibi Chibi Crystal Power, Make Up" is the command Chibi Chibi uses to transform into Sailor Chibi Chibi in the manga and crystal.

In Act 54, Princess Kakyuu correctly identifies Chibi Chibi as a Sailor Guardian and the child responds with a grin and calling out this phrase to transform for the first time. This surprises the Sailor Starlights. This is the only time she is shown using this phrase; however, she swaps between her Civilian and Sailor forms several times after this point.
Anime[]
Sailor Chibi Chibi Moon's Sailor Guardian form was seen in the anime, but she never had an actual transformation sequence or phrase. In episode 187, The Shining Power of a Star: Chibi-Chibi's Transformation, she transforms for the first time when she touches the Eternal Tiare and instantly becomes a Sailor Senshi. In episode 195 she seemed to be in mid-transformation when she appears before Princess Kakyuu.

- This is the only "Crystal Power" transformation which references a Sailor Crystal that doesn't exist - there is no "Chibi Chibi Crystal" as Chibi Chibi and Sailor Chibi Chibi are only disguises for another Sailor Guardian who has her own Sailor Crystal. In practice, it's similar to Sailor V and Sailor Moon's disguise phrases.
